We are delighted to welcome you to New Paradigm Holistic Mental Health Retreat and are committed to providing you with a safe and supportive environment to focus on your mental health and wellness.
Before we confirm your participation, we kindly ask that you complete our intake procedure. This process is designed to ensure that our retreat is effective and appropriate for all participants. It helps us understand your individual needs and goals, and confirms that our retreat is the right fit for you.
The intake procedure consists of five (5) steps. Please work through them in order. Our team is available at every step to answer questions and support you.

Emotional Evaluation
The emotional evaluation assesses your emotional needs and assists with identifying goals for treatment during your time at our mental health retreat. Please complete this form to the best of your ability and be as honest as possible.

Consultation(s)
We speak with you one to two times to talk about your current situation, the results of the emotional evaluation, and how we can help you. We will discuss a treatment plan, and determine if our methods of treatment and facilities are coherent with your expectations.

Rules & Guidelines Document
Please review our 25-point Rules and Guidelines document carefully. Once you have read it, we will send you the digital acknowledgment via DocuSign to sign. Let us know if you have any questions or concerns before signing.

Pay Deposit (50%)
We require the payment of a deposit of 50% to secure your reservation, as we only have 4 openings at one time. The outstanding amount is due on arrival and prior to the start of your program.
Payment instructions will be provided by our team once the previous steps are complete.
